  1. Complete an Application for Admissions. Forms are available at the Admissions Office, Room S041, or online.
  2. If you don’t have one, obtain a Social Security Number.
  3. Provide proof of immunizations.
  4. Provide Proof of Texas Residency for those who wish to be classified as residents for tuition purposes.
  5. Complete Texas Success Initiative requirements.
  6. Obtain a Brookhaven College student identification card from the Student Programs And Resources Office, Room S201.
  7. Provide proof of current CPR certification to the American Heart Association Healthcare Provider Level OR American Red Cross Professional Rescuer Level.
  8. Attend an EMS Program orientation day.
  9. Obtain the required textbooks.
  10. Obtain the EMS Kit(s) by the first day of class.
  11. Obtain required liability insurance policy.
  12. Consent to a thorough criminal background check at the student’s expense.
  13. Consent to a random drug screen at the student’s expense.
