Leave Forms and Attendance Reports

Supervisors must route all Application for Leave forms directly to the College Human Resources Office. (See Business Office Procedures Manual, sec. 3.6.0, for additional information about processing these forms.)

Eligible employees receive a computer generated report each month indicating their accrued, used and available leave.

Should an error be noted on the report, the employee is responsible for reporting the error to the Human Resources office. Questions regarding the report should be directed to Human Resources, Ext. 4813.
