Veterans Benefits: Certification

I am a Hazlewood student at BHC that is a former Texas member of the U.S. Armed Forces.

• Ensure you meet the Requirements for Eligibility.
• Fill out the application that fits your situation:
  Application for Veterans New to Hazlewood
  Application for Previous Hazlewood Recipients
• Hazlewood transcripts are required from all institutions at which you have used Hazlewood since Fall 1995.
• DD214 - Parent's member four or long form which includes discharge information.
• Tuition Account Summary or a copy of your CE sign up sheet completed - This is required every semester in which you desire to use your Hazlewood benefits.
