Nursing Application Checklist

  1. Submit an application for admission to Brookhaven College (if not already enrolled).

  2. Complete the Texas Success Initiative requirements

  3. Complete the Online Nursing Information Session

  4. Transfer Students must have completed a transcript evaluation with the Nursing Department and may need to submit course substitutions by the deadline indicated on the Admission Schedule and Deadlines page. (Course substitutions will be needed for all courses not matching the BHC curriculum plan). This includes courses taken at other DCCCD campuses.

  5. Complete the four prerequisite courses with a "C" or better in each course and a cumulative GPA of 3.0 or higher. 

  6. Completed Application Packet 
    The Nursing Program application packet includes various forms and documentations. Forms can be downloaded from the Application Packet page. Your packet will contain the following:
    • Nursing Program Application
    • Statement of Student Responsibility Form
    • Brookhaven College/Texas Board of Nursing Information Form

    • Note: Effective May 21, 2019 all applicants are required to have a positive titer for MMR and Varicella
    • Immunizations form with current physical exam signed by school nurse, Mildred Kelley, RN in the Health Center (Bldg. S, Room S072). Read more information about the required immunizations
    • Proof of an "A" in a Dosage Calculation class OR a copy of Dosage Calculation Workshop score with a minimum grade of 90%
    • Nursing Information Session Verification Form
    • Ranking for Admission Form
    • HESI A2™ Cumulative Score Sheet
    • Eligibility Requirements for RN Licensure in the State of Texas Form
    • Bring your original social security card and your current, valid Texas driver's license
