Steps For the Nursing Applications

Checklist - Steps to Join the Nursing Program

  1. Submit an application for admissions to Brookhaven College:
    • Submit application on line or in person
    • All official transcripts must be mailed or brought into the Admissions office (Bldg. S, Room S052) in a sealed envelope
    • Proof of residency and current Meningitis immunization must be brought to the Admissions office

  2. Complete the Texas Success Initiative requirements

  3. View the on-line Nursing Information Session

  4. Transfer Students must have completed a transcript evaluation with the Nursing Department and may need to submit course substitutions by the deadline indicated on the Nursing website. (Course substitutions will be needed for all courses not matching the BHC curriculum plan). This includes courses taken at other DCCCD campuses.

  5. Complete the four prerequisite courses with a "C" or better in each course and a cumulative GPA of 2.75 or higher.

  6. Forms to include in your packet:
    • Application to Nursing Program
    • Statement of Student Responsibility
    • Effective Oct. 1, 2016, all nursing students applying to the Nursing program are required to submit a positive titer, with their application, for the following immunizations: *MMR, Varicella and Hep B surface antibody. Also required: Negative TB test, tDAP, Meningitis and Flu Shot.
    • Proof of an "A" in a Dosage Calculation class OR a copy of Dosage Calculation Workshop score with a minimum grade of 90%
    • Nursing Information Session Verification Form
    • Ranking for Admissions
    • Copy of HESI A2 Cumulative Score Sheet
    • Eligibility Requirements for RN Licensure in the State of Texas Form
    • Critical Thinking Exam Score
    • Brookhaven College Board of Nursing Information Form
    • Bring a copy of your current social security card AND Texas State Driver's License
    • Completed Immunization Form signed by campus nurse, Ms. Mildred Kelley RN in Bldg. S, Room S072
